This feature allows you to add Internal Item Notes on an Item-by-Item basis that will be displayed by hovering over the info icon in the Item Name on Quote Create and Quote Edit:

This can be used to manage upsell opportunities or act as a suggested selling guide for your sales team by simply clicking on the "Settings" tab when creating or editing an Item to add the appropriate text in the "Show Internal Item Note" field as shown below:
